A membership application form is available below. The form is an Adobe Acrobat file that you can submit by fax or mail. Instructions are on the form, along with descriptions of our three membership categories.
Applicants for corporate or political entity memberships must fill out page two of the form so that we can provide maps on which you can outline the general locations of your underground facilities. This information will be used to computerize your facility locations.
Applicants for sustaining memberships should not request maps and can leave page two blank.
Current members can update their contact information using the Member Update Form.
